The Supervisor of Case Management
will provide direct supervision to Case Management staff. This position will work with the Chief Medical Officer, Behavioral Health Medical Director, and designees to support staff and provide clinical oversight and clear directives of objectives and goals for the assigned team. This position will assess the information and tools needed to measure outcomes through report analysis. This position serves as the primary liaison between front line staff and management to communicate daily activities and departmental progress toward meeting goals.
Along with other leaders, this position will have responsibility for the submission of the Model of Care (MOC), and any Service Area Expansion as assigned and will assist in developing systems to document transitions activities in Viva’s EHR. This position may travel to locations within the Viva Health service area through a reliable means of transportation insured in accordance with Company policy.
•Graduate of a professional program of nursing, ADN/BSN
•5 years' experience in Acute, Post Acute, or Behavioral Health clinical setting
•Current RN license in good standing with the State of Alabama Board of Nursing
•Valid driver's license in good standing
•Experience in Chronic Disease Management
•Certified Case Manager (CCM)
