Will work at both St Vincent Fishers - 13861 Olio Rd - Fishers, IN & St Vincent Anderson 2015 Jackson St. - Anderson IN

As our Case Management Registered Nurse,

you will make a difference in patients’ lives by coordinating their healthcare needs using internal and external resources.  You will be an integral member of the healthcare team ensuring that the patient and team are on track with the care plan

What You Will Do

As a Case Manager with St. Vincent Health-IN, you will have the opportunity to coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.

Responsibilities:

Creates plan for care across the continuum, integrating patient/family preferences and values.

Monitors patient care through assessments, evaluations and/or patient records.

Advocates for resources and removal of barriers.

Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

What You Will Need

Licenses/Certifications/Registration:

Required Credential(s):

One or more of the following:

Registered Nurse credentialed from the American Nurses Credentialing Center (ANCC) obtained prior to hire date or job transfer date. 

Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date. 

Preferred Credential(s):

BLS Provider credentialed from the American Heart Association (AHA). 

Education:

Diploma 

Diploma from an accredited school/college of nursing required

Work Experience:

RN Case Management experience preferred and/or 5 years RN hopsital clincal experince.
